U.S. General Hospital Fort Scott Kansas January 12th 1864

I enclose herewith the blanks filled no as requested in your communication of Dec 22nd 1863 to a General Hospital May 24th 1862. The records previous to this date are not complete and it is not possible to give any correct information from an earlier date. In the numerical list I give you both those who were admitted to beds in hospital and those who reported to the hospital and were treated in quarters.
